be emailed to the Project Coordinator. This form is slightly different
from the one used before, which now requires that you login. As a result,
your data will be entered directly into the IHAD Database, eliminating
a large number of data entry hours.
Username:
Username is the first letter of your first
name followed by last name (eg John Doe is jdoe)
There are no spaces
The maximum username length is 16 characters
They are all lower case
Password is your birthday (mm/dd/yyyy) security number
